Welcome to the Student Enrolment Data Upload Portal
The Primary School Nurse Health Readiness Program (Prep Vision Screening) is a 'school health program’ as defined by the Public Health Act 2005, allowing the service to request student and parent/guardian information from school principals. This enables the program to support schools and families with the consent process and other vision screening queries. For more information, please visit our website:

This is how to upload a file:

Click the 'Choose File' button above and select a single Excel .CSV file from the picker pop-up with a file size less than 2MB.

Click the Upload button to upload your selected file.

Once uploaded, the file details will appear below the Upload button.
A green tick
will be displayed next to the file name to confirm a successful upload.
If you experience difficulties in uploading the Student Enrolment data or get a red cross beside your uploaded file, please contact the Prep Vision Screening team immediately for assistance.
